Dengue is one of the most well-known viral diseases worldwide, especially in countries endemic to tropical diseases. It presents a heterogeneous clinical spectrum, ranging from asymptomatic infections to severe cases with multiorgan failure. This case involves a male patient who presented to the emergency department with fever spikes of 3 days' duration and signs of severity compatible with multiorgan dysfunction secondary to severe dengue. The patient showed a significant elevation of transaminases and, after ruling out other probable etiologies, was diagnosed with fulminant hepatic failure. Hepatic involvement should not be overlooked, highlighting the importance of suspecting this condition to make an early diagnosis and promptly initiate appropriate treatments. Managing dengue requires special care and attention, as it is a dynamic disease; therefore, it is necessary to evaluate markers of liver damage and function in patients with dengue.
