Neuropsychiatric symptoms in a patient with cirrhosis: beyond hepatic encephalopathy

In 1914 a set of neurological symptoms associated with severe liver disease (cirrhosis) were described, which were not clearly elucidated until their complete anatomopathological description in 1965, which allowed its differentiation from Wilson's disease - the main differential diagnosis -, being recognized then as acquired hepatocerebral degeneration (HCD). It has rarely been reported and the available literature comes up short for accurately explaining this entity, characterized by movement disorders and cognitive impairment. So that, the case of a 74-year-old patient with cryptogenic liver cirrhosis since the age of 50 is presented. She attended the outpatient service of a high-complexity hospital in Medellín, Colombia, referring a group of motor and cognitive symptoms. This case required a multidisciplinary, paraclinical and imaging approach to determine the correlation of the clinical picture with HCD, representing a diagnostic challenge due to the limited knowledge about the entity.
